Consultation Form
Persons seeking legal representation with our law firm may wish to schedule a telephone or in-person consultation with our attorneys. In order to do so, we request that you complete the Immigration Consultation Questionnaire and send it to our office by clicking "Send Questionnaire" at the end of the form. If you wish, you may fax the questionnaire and any other documents which are relevant to your case to us at (818) 907-8471.
The faxed information and documents should pertain to the person who is seeking a temporary visa, permanent residence, citizenship, relief from deportation or any other immigration benefit. If you are an employer seeking to sponsor one or more employees, a separate questionnaire should be completed for each employee. If you are a citizen or permanent resident seeking to petition for and/or sponor a foreign relative, the information contained in the form should be about your relative.
There is no charge for an initial consultation. However, consulting with one of our attorneys does not establish an attorney/client relationship.
All information contained in the Immigration Consultation Questionnaire will be kept STRICTLY CONFIDENTIAL and will not be released without your permission.
